From the News-Times (Hartford City):

Q: Do political ads with the disclaimer “Paid for by the Committee to Elect John Doe” also have to include an address?

A: The statute doesn’t require an address for the committee in a political ad. The committee has to register with either the county election board or the state election division of the Indiana Secretary of State’s Office.

That address information is publically available, but it doesn’t have to be included in an advertising disclaimer.
